The
MBSS can be scheduled in a timely manner
upon receipt of the Quest Imaging Paperwork:
1. Patient Demographic Sheet (Patient Information)
2. Facility Face Sheet
3. Copy of the Physician’s Order
4. Patient Authorization/Billing Agreement
(Giving
permission to bill insurance for the MBSS)
Quest
Imaging prefers to provide MBSS
services to facilities under a contract;
however a contract is not necessary in order
to receive services. For more information on a
contract, please call the Quest corporate
offices.
